You will be guided in a step by step process to identify wellness goals, assess your specific impairments, implement a plan to improve your health and resume your functional activities.
Wisconsin is a Direct Access state, which means you do not need a referral to get treatment from a Physical Therapist.
However, if you plan to submit costs for reimbursement, it is advised that you check with your plan as they may require a Physician’s order for coverage.
Badger State Health is an Out of Network Provider (OON). Your health insurance plan may or may not provide some OON benefits. It would be best to clarify with your insurance provider whether your plan covers these types of costs and at what level. We will provide a detailed invoice for you to submit to your insurer, although coverage is not guaranteed.
We will accept HSA/FSA funds, Venmo, Cash, or Credit to pay for your visit.
Initial Evaluations and Subsequent Encounters are all 1 hour long.
In all cases, it is best to wear comfortable clothing that is appropriate for exercise. We will be assessing your strength, range of motion, mobility and incorporating exercises in each session. Some sessions will be dedicated more toward Dry Needling and Manual treatments.
